The following are answers to various questions that we receive often.
Q: Do I need a referral to come to Millbrook Physical Therapy?
A. You do not need a referral or prescription to come to physical therapy. Anyone can see us for either 10 visits or 30 days for an injury/condition without a doctor’s referral or prescription. After this period, a prescription is needed (this does not apply to our injury prevention/performance enhancement programs). Insurance coverage for this 10 visit/30 day period varies, so be sure to contact your insurance provider to check on your coverage. You are responsible for paying in full for your sessions at the time of service for whatever amount your insurance does not cover.
Q: Prescription vs Referral
A. A prescription is a note from the Physician to the Physical Therapist with a diagnosis recommending physical therapy treatment. This avoids the 10 visits / 30 day rule and allows us to see you longer. A referral is an insurance document that is sometimes needed by the insurance company to receive covergae for a visit.
Q: What insurances do you participate with?
A. Please see our Insurance Information.
Q: What should I bring to my initial visit?
A. Please bring your insurance card and driver’s license to your initial visit. You may also download and complete our patient forms in ourForms Section to save yourself time. If your insurance requires a physician’s persecription or referral in order to cover payment for your sessions, please be sure to bring this is well. It is also recommended that you dress in comfortable clothing that will allow your clinician to access the body part(s) to be evaluated and treated.
Q: How can I pay for my visit?
A. Please pay necessary session costs at the time of service. We accept cash, check, Visa, or MasterCard.
Q: How will my doctor know how I’m doing with therapy?
A. We send regular updates to your physician, giving them a clear picture of how you are responding to therapy. Please be sure to keep your clinician informed of any scheduled doctor’s appointments, so that he/she can provide an update at that time.
